CALL FOR CLINICS

The Florida Dental Laboratory Association Board of Directors is calling for technical/scientific clinics and presenters from the dental laboratory industry to be considered for the 2012 Symposium & Expo.

Please click here to download the 2012 Call for Clinics Forms and upon completion, submit to the FDLA headquarters with a clinic description, presenter’s bio, and presenter’s headshot.  The clinic description, presenter’s bio and headshot can be emailed to jillian@fdla.net. 
The Call for Clinics document should be faxed to (850) 222-3019 or mailed to Jillian Heddaeus, CMP at the address below.

2012 Call for Clinics All Forms

All applications must be submitted to the FDLA office NO LATER November 1, 2011 to be considered.

Applicants will be notified of the selected clinics and presenters by January 6, 2012.  Any Call for Clinics form that is not fully completed, or not submitted with the required supporting documents (clinic description, presenter’s bio and headshot) will be returned for further information.  Submitting a Call for Clinics form does not guarantee placement at the 2012 Southern States Symposium & Expo.  The FDLA Conference Committee & Board of Directors will make all final decisions in regards to clinic selection and times.
